CHRONIC DISEASE MANAGEMENT
At Ashgrove West Group Practice we have a team-based approach to chronic disease management. Our Registered Nurses, in collaboration with the GPs, are involved in providing the following services:
- Health Assessments – 75 years and over, 4 year old Healthy Kids
- Check, 45-49 years Health Check
- GP Management Plans and Team Care Arrangements
